چکیده

The authors present the case of a 54-year-old female who developed a painful compression lesion localized to the medial aspect of the base of the distal phalanx of the great toe as a complication of hallux valgus surgery. Preoperative radiographic evaluation of the patient’s foot revealed the first ray to be longer than the second, a 12° first intermetatarsal angle, a 33° hallux abductus angle, and an exostosis at the medial aspect of the base of the hallux that was not considered by the surgeon to be important. Correction of the hallux valgus deformity was performed with a combination of scarf and Akin osteotomies, and the intermetatarsal and hallux abductus angles reduced to 2° and 8°, respectively. By 2 months postoperative, the patient was complaining of pain at the medial aspect of the distal phalanx of the hallux associated with shoe pressure. The pain correlated both clinically and radiologically with the exostosis at the base of the distal phalanx, and had become symptomatic only after the hallux had been operatively realigned. At 6 months postoperative, percutaneous exostectomy was undertaken to remove the exostosis. Pain relief was complete, thereafter, and after 2 years of postoperative follow-up the patient remained pain free. The clinical importance of a medial exostosis localized to the base of the distal phalanx of the hallux must be taken into consideration whenever hallux valgus correction is undertaken, and this is particularly important whenever an Akin osteotomy is being considered. Level of Clinical Evidence: 4

An Unusual Presentation of Chondrosarcoma of the Clavicle With Horner's Syndrome

We report a unique case of symptomatic chondrosarcoma of the medial end of the clavicle resulting in pain and Horner’s syndrome. To our knowledge this is the first reported case in the literature of Horner’s syndrome secondary to a clavicular tumour. Osteochondroma, a frequent benign tumour of the bone, is in many cases asymptomatic. The reported risk of malignant transformation in a solitary o...

Extensive Osteochondroma of Talus Presenting as Tarsal Tunnel Syndrome: Report of a case and Literature Review

Osteochondroma or exostosis is the most common benign bone tumor, and occurring frequently in the proximal humerus, tibia, and distal femur. It rarely affects talus. Osteochondroma of talus is a very rare etiology of tarsal tunnel syndrome (TTS). We report a rare case of extensive osteochondroma of the talus in a 60 year old female presenting with multiple swellings around the ankle and sympt...

The Clinical Outcome of Simultaneous Lateral Closed- Wedge Distal Femoral Osteotomy and Anterior Cruciate Ligament Reconstruction in the ACL-deficient Knees with Symptomatic Femoral Varus Deformity

Background: Nowadays combined high tibial osteotomy and ACL reconstruction is accepted as a safe and effectivesurgery for patients with symptomatic varus osteoarthritis and anterior knee instability; however, the source of varusdeformity is sometimes the femoral bone. No studies have reported concomitant ACL reconstruction and distal femoralosteotomy in ACL-deficient knees wit...

Subacromial impingement syndrome in a patient with hereditary multiple exostosis: a case report

BACKGROUND Hereditary multiple exostosis (HME) is characterised by multiple osteochondromas that are distributed throughout the skeleton, invariably involving the shoulder girdle. Tumours within the subacromial space can cause secondary irritation of the rotator cuff and result in subacromial impingement syndrome. CASE PRESENTATION We describe a 19 year old female patient with HME who present...

BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2013202273 DESCRIPTION Osteochondromas (exostosis) are commonly found benign tumours (35–46%) of the bone. 2 These are mostly found in the metaphyseal region of long bones and the scapula is involved rarely in 3–4.6% of cases. These scapular tumours may present with winging of t...
